Patient Information
Financial and Insurance
Our office is committed to providing the best medical care for your children. We are pleased to discuss our professional fees with you at any time. Your clear understanding of our financial policy is important to our professional relationship. Please ask if you have questions about our fees, or financial policy, or your financial responsibility.
Participating Insurance Plans
We require you to show your child's current insurance card at each office visit. All copays are due the day your child is seen when you check in for the appointment. If your insurance company does not respond to our claim's submission the balance will become your responsibility. Balances are due within 30 days of the billing. If your coverage was terminated and you do not present us with the new insurance card within 30 days of the date of service, the bill is your responsibility. If your plan does not require a copay and we participate with the plan, we will submit the bill to your insurance company. You are responsible for any deductibles and balance your plan indicates on their EOB (explanation of benefits). These balances are due in full within 30 days of your first billing. Certain services we provide may not be reimbursed by your insurance company and will become your financial responsibility.
CASH PAYMENT AND NONPARTICIPATING INSURANCE PLANS
If you have no insurance or if you have insurance but we are not network providers for your plan, our fee is due at the time the child is seen. We believe that keeping your child healthy shouldn't be a financial burden and that is why we offer a variety of payment options to make your child's treatment more affordable. For your convenience we accept Visa, MasterCard, Discover and American Express. We offer a 20% discount if payment is made in full at the time of your appointment.
DIVORCE SITUATIONS
We do not have legal authority to enforce divorce decrees. Therefore, the parent who brings the child in for their visit is responsible for payment at the time of service regardless of the financial arrangement of the divorce agreement.
EVENING AND WEEKEND CHARGES
There is a small additional charge for medical services provided Monday through Friday after 5:00 pm, and on Saturday morning. There is a larger additional charge for medical services provided after office hours, on Sundays, and on Holidays. Most insurance companies cover some or all of this fee, but some do not and it will become your responsibility to pay if the insurance does not.
PATIENT ACCOUNTS
You are responsible for the timely payment of your account. You will receive an initial bill a few days after your visit in our office. If we do not receive payment you will receive a follow-up bill 28 days later. If you have not made a payment after 60 days you will receive a letter from us requesting immediate payment of your bill. Any patient balance left unpaid after that without any attempts on your part toward resolution will be considered delinquent and will be submitted to a collection agency. If you are having financial hardship please speak with the billing office and we will make every effort to set up an acceptable payment plan with you. If your account is turned over to collection, we will continue to provide care for your children on a cash at time of service basis. You will be required to pay for the visit when you check in for your appointment. The 20% cash discount does not apply for accounts that have been sent to collections. Submission of your account to collection may adversely affect your credit rating.
